    Hoola Lahui Hawaii

    Hoola Lahui Hawaii's (HLH) mission is to preserve and perpetuate the Hawaiian race by achieving community wellness through innovative and culturally appropriate health services and education programs. The organization serves the island of Kauai and individuals from Niihau. Established as one of five federally funded Native Hawaiian Health Care Systems, Hoola Lahui Hawaii provides care to Hawaiians and their families with a primary focus on health promotion and prevention. As of September 2001, HLH is also a section 330 community health center.

    Special Populations/Health Issues
    Native Hawaiians; immigrants. As Kauai has a small island population, the number of services available is limited for obvious economic reasons. Additionally, many Hawaiians do not feel comfortable receiving care from western medical practitioners.

    Services
    HLH provides health services including comprehensive dental services and limited primary care services on its mobile health outreach van. The agency also provides programs in health promotion, disease prevention, health education, nutrition, exercise, and chronic disease management.

    Patient Profile
    Since 1992, Hoola Lahui Hawaii has served more than 8,000 individuals and continues to serve some 1,200 annually. As funding is primarily geared towards Hawaiians, HLH targets this population and their families. However, services are open to anyone based on a sliding fee scale. HLH continues to serve the uninsured with over 65% of their clients having no insurance.

    Staffing
    HLH staff includes physicians, dentists, registered nurses, community health workers, health educators, dental assistant, certified fitness instructors, and administrative staff.

    Kauai Community Health Center
    Overview
    Kauai Community Health Center (KCHC) is Hawaii's newest community health center. In 2000, Hoola Lahui Hawaii—the Native Hawaiian Health System for Kauai and Niihau—applied for federal funds to open Kauai's only community health center. HLH received the funds and opened KCHC in Waimea in November 2001. (For information on Hoola Lahui Hawaii, the KCHC parent organization, please see section on Native Hawaiian Health Care Systems.)

    Services
    KCHC provides comprehensive medical services, dental services, family planning, perinatal, nutrition, behavioral health, preventive services, health education, eligibility assistance, language interpretation, case management and outreach.

    Special Populations/Health Issues
    Low-income families, Native Hawaiians, immigrants, uninsured and under-insured. Health indicators suggest problems typical of low-income Asian and Pacific Island communities. They include high rates of asthma, diabetes, obesity, vaccine-preventable diseases, cardiovascular disease, substance abuse, and dental disease. Problems of access include low per capita numbers of primary care providers, limited health facilities, a low number of physicians and dentists willing to accept Medicaid patients, a 7% rate of uninsured for the service area, a 29.4% uninsured rate for the target population, and a severe shortage of substance abuse treatment services.
